Pathway Plan - Your Voice

Introduction

As a foster carer to a young person in care it is important that you are able to contribute fully to their Pathway Plan Review and to identify how things are going for the young person, you and your wider family. The new Pathway Plan Review means that you will be doing this in two key ways. The first is the letter that you will write to the young person every six months where you will write of their key events, achievements and magic moments in their life. This letter is intended for the young person and personal to your relationship with them.

Alongside this letter it is important that you have the opportunity to reflect on other aspects of the young person’s care in the review period. You will need to think about the young person’s needs, your views about their care plan, how the adults around the young person have worked together to meet their needs and if there is anything that you think might help them now or in the longer-term. You can record your views about these things here and share them with the Independent Reviewing Officer (IRO) and/or social worker during the Pathway Plan Review process. You can use the form flexibly, only filling in the parts that are relevant to you.
